The Medicaid Waiver Division of Three Rivers Planning & Development District, Inc. offers Long-Term Care Alternatives and Home and Community Based Services to eligible individuals. Each county has a “Medicaid Waiver Team” which consists of a Registered Nurse and a Licensed Social Worker who is responsible for setting up and brokering services for clients, inclusive of monitoring, assessing, and reviewing clients plan of care monthly in the home of the client and amending the plan of care to meet the needs of the client and his/her caregiver, if applicable. Each team services an average of 100 clients each month.
